Health Insurance

Medical insurance options are provided through UMR, a United Healthcare company. We offer two high-deductible health plan options combined with a tax-advantaged Health Savings Account. All coverage options offer prescription coverage and 100% coverage for preventive care and certain cancer screenings. 1st Source pharmacy benefit includes an enhanced list of preventative maintenance medication list.

Maternity Care Program

Voluntary Maternity Care Program offered through UMR helps you develop healthy lifestyle choices and access proper medical care before and during your pregnancy. Offered to eligible members at no additional cost as part of the benefit plan.

Real Appeal Weight Loss Program

Real Appeal is an online voluntary weight loss program that is offered to eligible members at no additional cost as part of the benefit plan.

Telemedicine

Telemedicine gives you 24/7/365 access to U.S. board-certified doctors through the convenience of phone, video or mobile app visits. It’s an affordable option for quality medical care.

Tobacco Cessation Program

Our Tobacco Cessation Program is a voluntary program that is offered to eligible members at no additional cost as part of the benefit plan.

Dental Insurance

Dental insurance is provided by Delta Dental and offers comprehensive dental coverage. In-network preventive care is covered at 100%.

Vision Insurance

Vision insurance is provided by UHC Vision and offers discounted care from in-network providers for eye exams and glasses and/or contact lenses.

FINANCIAL WELLNESS

401(k) Plan

401(k) plan includes both Traditional (before-tax) and Roth (after-tax) savings options. 1st Source matches dollar for dollar on the first 4% and $0.50 per dollar on the next 2%. Diverse investment options and investment advice are available. Match is subject to board approval.

2% Annual Employer Contribution

The Company will contribute 2% of your eligible earnings on an annual basis. Subject to board approval.

Discretionary Profit Sharing

The Company will contribute a discretionary amount on an annual basis. Discretionary profit-sharing contribution. Subject to board approval.

Health Savings Account

Health Savings Accounts (HSA) allows you to use pre-tax dollars to pay for eligible health care expenses (as defined by the IRS) that are not paid by a 1st Source medical, dental or vision plan (or any other plan under which you or your eligible dependents are covered).

Flexible Spending Account

A flexible spending account (FSA) allows you to set aside pre-tax dollars from your paycheck to cover qualified medical, dental, vision, and dependent care expenses you would normally pay out of your pocket. We offer three types of FSA programs.

  • Dependent Care FSA

Disability Insurance

1st Source offers company-paid short-term and long-term disability insurance if you become unable to work due to illness, injury, physical condition (including pregnancy and pregnancy-related), or mental condition.

Life Insurance

Life and accident insurance provides different levels of coverage to help protect you and your family from the financial impact of an unexpected loss of life or an accident.

Coverage includes basic life, voluntary life and coverage for your dependents.

Identity Theft Protection

Identity theft protection, offered at a discounted rate, provides credit reports, scores, and alerts, internet fraud monitoring, and identity fraud resolution services.

Stock Purchase Plan

Tuition Assistance

We support and encourage professional development for colleagues who wish to continue their education for personal growth and to enhance their professional careers. Tuition expenses are covered up to a maximum of $6,000 per year for full-time employees. Part time employees will be covered up to a maximum of $3,000 per year (including expenses for technology and lab fees, and books).
